Responsibilities

  • Provide resident-centered nursing care within Colorado LPN scope of practice, including assessments, monitoring, and supportive clinical interventions.
  • Conduct routine and as-needed health evaluations; identify changes in condition and notify the RN, provider, or supervisor as required.
  • Administer medications and treatments in accordance with physician orders, facility policy, and state regulations.
  • Assist in developing, updating, and implementing individualized care plans in collaboration with residents, families, and care staff.
  • Oversee medication administration practices, including medication documentation, reconciliation, and storage compliance.
  • Support personal care staff (PCPs/QMAPs) through guidance, coaching, and ensuring adherence to care standards.
  • Respond to resident incidents, emergencies, and illness episodes, documenting and following reporting procedures.
  • Maintain accurate and timely clinical documentation, including assessments, interventions, communication, and follow-ups.
  • Communicate effectively with residents, families, physicians, and external care partners to support continuity of care.
  • Ensure infection control, safety, and regulatory standards are followed throughout the community.
  • Participate in shift report, care conferences, training, and quality improvement initiatives.

Qualifications

  • Current, unrestricted Colorado LPN license.
  • Current CPR/BLS certification (facility-specific).
  • Competency in medication administration, nursing assessment, and basic clinical procedures.
  • Ability to interpret physician orders, follow care plans, and document care accurately.
  • Strong interpersonal communication skills and ability to work collaboratively with caregivers, residents, and families.
  • Ability to meet physical demands: lifting up to 35–50 lbs, assisting with mobility, and extended standing/walking.
  • Ability to pass all required background checks, including Colorado Bureau of Investigation (CBI) and any applicable federal screenings.

Pay: $ $35.00 per hour
