What You'll Do

As a Home Health Physical Therapist, you'll help patients regain mobility, confidence, and independence through personalized, one-on-one care in their homes.

• Conduct physical therapy evaluations and OASIS assessments
• Develop and implement individualized care plans to restore function and mobility
• Deliver hands-on therapeutic interventions and home exercise programs
• Educate patients and caregivers on proper exercise, movement, and safety techniques
• Collaborate with nurses, physicians, and other care team members to optimize outcomes
• Maintain accurate and timely clinical documentation

What You'll Bring

• Active New Jersey Physical Therapist (PT) license
• Master's or Doctorate in Physical Therapy
• Home care experience (required)
• CPR certification, valid driver's license, reliable transportation, and insurance
• Excellent communication and organizational skills
• Passion for providing personalized, in-home rehabilitative care
