Job Summary
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will provide critical support and guidance to individuals facing behavioral health challenges, addiction issues, or social service needs. This role involves coordinating care, developing individualized plans, and connecting clients with community resources to promote stability and recovery. The Case Manager will work closely with diverse populations, including children and families, ensuring they receive the necessary services to improve their well-being and achieve their goals.

Responsibilities

  • Conduct comprehensive assessments to identify client needs related to behavioral health, addiction, child welfare, or social services.
  • Develop personalized care plans in collaboration with clients, family members, and multidisciplinary teams.
  • Provide crisis intervention and crisis management to support clients during urgent situations.
  • Deliver addiction counseling and motivational interviewing techniques to motivate behavioral change.
  • Coordinate services such as Section 8 housing, Medicare, childcare programs, and workforce development initiatives.
  • Monitor client progress and adjust care plans as needed to ensure positive outcomes.
  • Maintain detailed documentation of case activities, service plans, and progress notes in compliance with agency policies.
  • Supervise or mentor junior staff or interns as part of team development efforts.
  • Advocate for clients within healthcare systems, social services, and community agencies to facilitate access to resources.

Qualifications

  • Proven experience in crisis intervention, crisis management, or behavioral health settings.
  • Knowledge of addiction counseling methods and social work principles.
  • Strong understanding of child welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives.
  • Excellent communication skills with the ability to motivate and mentor clients effectively.
  • Supervising or mentoring experience is preferred.
  • Background in childcare or mentoring is a plus.
  • Ability to handle sensitive situations with professionalism and empathy.
  • Relevant educational background in social work, psychology, counseling, or related fields is highly desirable. This position offers an opportunity to make a meaningful impact on individuals' lives through dedicated service and community engagement.
