Position Purpose:

The Youth Transition Case Manager provides individualized case management, advocacy and support for youth transitioning out of care. This position helps clients achieve stability in housing, education, employment, healthcare, and life skills development through a trauma-informed and strength-based approach.

Key Area's and Task Results:

  • Provide case management services to youth/young adults aged 16-25
  • Develop and implement individual youth transition plans in collaboration with youth and support networks
  • Assist youth in obtaining and maintaining housing, employment, education, healthcare and other essential services
  • Support youth in learning independent living skills such as budgeting, cooking, time management, and navigating systems.
  • Conducting regular home visits, community outreach and in-office meetings.
  • Maintain accurate and timely documentation and case notes.
  • Provide crisis intervention and problem-solving supports as needed.

Relationships and Supervision:

  • Reporting directly to the Director of Splatin Stsmamlt Services.

Qualifications:

  • Experience supporting youth or young adults navigating social systems.
  • Experience working with Indigenous children, youth, and families.
  • Knowledge of Splatsin community and culture is valued.
  • Understanding of trauma, resilience, and adolescent development through professional or personal experience.
  • Knowledge of community resources and services.
  • Strong relational and communication skills.
  • Ability to work independently and manage a flexible schedule, including some evenings and weekends.
  • High ethical standards
  • Experience with case management systems/client databases.
  • Experience with MS Office.
  • Must have a valid driver's licence, reliable transportation and ability to travel locally.
  • Clean Criminal Record Check and Vulnerable Sector Check.

Job Type: Full-time

Pay: $27.00-$37.00 per hour

Expected hours: 35 per week
