This is not a typical NP role. Aiyana Services is seeking a high-impact clinical leader who will serve as the linchpin between Edmonton's Somali community and quality healthcare. You will build something meaningful, an integrated care model that breaks down language barriers, bridges cultural gaps, and delivers measurable health outcomes for an underserved population.

What You Will Accomplish in Your First Year

These are the key performance objectives that define success in this role:

  • Establish Integrated Care Pathways: Within 90 days, develop and implement standardized care coordination protocols connecting primary care, behavioural health, and community resources, resulting in measurable reductions in emergency department utilization among your patient population.
  • Build Community Trust & Referral Networks: Cultivate relationships with 10+ community organizations, faith leaders, and healthcare providers serving Edmonton's Somali population, creating a robust referral ecosystem that increases service access by 25% within 12 months.
  • Deliver Culturally Responsive Clinical Excellence: Provide direct clinical care coordination for 150+ patients annually, achieving patient satisfaction scores exceeding 90% through culturally informed, evidence-based interventions addressing medical, psychological, and spiritual needs.
  • Champion Quality & Compliance: Lead quality improvement initiatives ensuring 100% compliance with Alberta Health Services standards, HIA (Health Information Act) requirements, and organizational policies, while maintaining impeccable clinical documentation.
  • Develop Clinical Education Programs: Create and deliver at least four educational workshops for patients, families, and caregivers on preventive care, chronic disease management, and mental health, tailored to Somali cultural contexts and health literacy levels.
  • Serve as Clinical Resource & Team Leader: Function as the go-to clinical nursing resource for consultation, mentorship, and interdisciplinary case conferencing, elevating the care team's collective capability to serve Somali-speaking patients.

What You Bring Essential

  • Active Nurse Practitioner registration with the College of Registered Nurses of Alberta (CRNA).
  • Fluency in Somali and English (spoken and written), you are the cultural and linguistic bridge.
  • Master's degree in Nursing from an accredited institution.
  • Minimum three (3) years of Nurse Practitioner experience, with demonstrated care coordination or case management capability.
  • Current BLS certification (Heart & Stroke Foundation or equivalent).

Preferred

  • Experience in integrated behavioural health, mental health, or community health settings.
  • Track record of building community partnerships or referral networks.
  • Hospital or inpatient care experience as a Nurse Practitioner.
