Looking for a 7A-7P and 7P-7A. 36 hours a week. Shift differential is $2.00 2nd shift $3.00 3rd shift and $5.00 for weekends.
*Responsibilities*
* Provide direct patient care, including administering medications and treatments as prescribed.
* Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
* Assist with daily living activities for patients, including personal hygiene and mobility support.
* Document patient care activities accurately in electronic health records (EHR) systems.
* Collaborate with registered nurses and physicians to develop and implement individualized care plans.
* Educate patients and their families about health management and disease prevention.
* Maintain a clean and safe environment for patients, adhering to infection control protocols.
* Participate in discharge planning and coordinate follow-up care as needed.
*Qualifications*
* Valid LPN license in the state of practice.
* Experience in critical care, long-term care, or acute care settings is preferred.
* Familiarity with medical terminology, ICD coding (ICD-9 & ICD-10), and EMR systems such as Epic or Cerner.
* Knowledge of patient assessment techniques and basic life support (BLS) certification is required.
* Experience working with diverse populations, including those with developmental disabilities, dementia, or Alzheimer's care.
* Strong communication skills to effectively interact with patients, families, and healthcare teams.
* Ability to perform procedures such as IV insertion, phlebotomy, catheterization, and medication administration.
* Understanding of geriatric care principles and familiarity with hospice or palliative care practices is a plus.
