Job Summary

CalVet Pharmacy is currently gathering a pool of highly qualified candidates for an anticipated opening for a Geriatric Pharmacist on our team. This role involves both working in daily pharmacy operations and providing expert clinical consultation to our CalVet Providers. You will contribute to the execution of the daily workflow and logistics, while also optimizing medication therapy through comprehensive Drug Regimen Reviews which can significantly impact the quality of care of our elderly veterans. We highly value work-life balance, which is reflected in the set, employee-friendly regular work schedule (Monday through Friday, during normal business hours) with NO required weekends, evenings, holidays or on-call responsibilities.

Responsibilities

  • Processing Orders: Perform PV1 (data entry/verification) and PV2 (final product verification) of dispensed medications for SNF and RCFE residents.
  • Perform Comprehensive Drug Regimen Reviews (DRRs): Conduct thorough, timely, and evidence-based clinical reviews for all assigned residents.
  • Physician Resource: Act as a clinical pharmacy resource for the medical staff, offering evidence-based advice and recommendations in formal meetings and upon request via telephone
  • Clinical Intervention: Collaborate with prescribers to develop and implement individualized medication therapy plans, emphasizing deprescribing and safety for geriatric patients.
  • Formulary Management: Maintain the pharmacy formulary, working with leadership and prescribers to ensure cost-effective and clinically appropriate drug choices.
  • Regulatory Compliance: Ensure all dispensing and operational practices comply with California State Board of Pharmacy regulations, California Title 22, and federal laws.
  • Quality Improvement: Lead or support facility-wide initiatives related to medication use, risk reduction, and continuous quality improvement (CQI).

Requirements

  • Education: Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited school of pharmacy.
  • Licensure: Valid and active CA Pharmacist License
  • Experience: Geriatric pharmacy experience providing patient care in long-term care settings, such as SNFs, RCFEs.

Additional Highly Valued Qualifications

These credentials are not required, but are highly recommended and will significantly enhance a candidate's application:

  • Advanced Clinical Training: Successful completion of an ASHP-accredited PGY1 Pharmacy Residency program is highly valued.
  • Board Certification: Current Board Certification in Geriatric Pharmacy (BCGP) or Board Certification in Pharmacotherapy (BCPS) from the Board of Pharmacy Specialties (BPS) is strongly recommended.
  • Experience with specific pharmacy operating systems and electronic health records utilized by long-term care facilities.
