Social Worker

Responsibilities:

• Conduct patient assessments to determine their needs and develop appropriate treatment plans
• Collaborate with healthcare professionals to coordinate patient care and ensure continuity of services
• Maintain accurate and up-to-date medical documentation and records
• Conduct diagnostic evaluations to assess patients' mental health conditions
• Implement evidence-based treatment modalities such as cognitive behavioral therapy
• Assist patients and families in end of life planning
• Provide bereavement and greif support
• Advocate for patients' rights and assist them in accessing necessary resources and support systems

Experience:

• Bachelor's or Master's degree in Social Work or related field
• Experience working in a home health or hospice setting is preferred
• Knowledge of special education laws and regulations
• Strong assessment skills to evaluate patients' mental health conditions
• Proficiency in conducting group therapy sessions
• Ability to work collaboratively with interdisciplinary teams
