Description of Work

The Early Intervention Service Coordinator (EISC) provides professional-level service coordination for infants and toddlers, birth to age three, who are enrolled in the North Carolina Infant-Toddler Program through the Sandhills Children's Developmental Services Agency (CDSA). This position ensures compliance with federal and state regulations by facilitating timely evaluations, developing and monitoring Individualized Family Service Plans (IFSPs), and coordinating early intervention services such as speech, occupational, and physical therapy. The EISC works closely with families, caregivers, and multidisciplinary teams to support child development within natural environments and daily routines. The role requires independent judgment, strong organizational skills, and frequent travel within the CDSA catchment area. The position plays a critical role in empowering families and enhancing developmental outcomes during a key period of early childhood growth.
Key Duties Include:
-Conduct initial contacts with families to explain the Infant-Toddler Program, obtain consents, and gather intake and financial information.
-Coordinate and participate in eligibility evaluations and assessments, ensuring procedural safeguards are followed throughout the process.
-Facilitate family-directed assessments to identify strengths, needs, and priorities that inform service planning.
-Develop and implement Individualized Family Service Plans (IFSPs) in collaboration with families and multidisciplinary team members.
-Authorize and coordinate early intervention services, including arranging provider access and ensuring service delivery aligns with IFSP outcomes.
-Monitor and review IFSPs regularly, adjusting services as needed and documenting progress toward developmental goals.
-Maintain accurate and timely documentation, including data entry, provider notes, and required reporting for program compliance.
-Support families in accessing community resources, understanding their child's development, and advocating for their needs.
-Provide consultation and technical assistance to community agencies and professionals involved in early intervention services.
-Plan and coordinate transitions for children exiting the program, including referrals and collaboration with receiving agencies or schools.

Knowledge Skills and Abilities/Management Preferences

Salary Grade Range: $35, $62,971.00 Recruitment Range: $35, $52,055.00

The Knowledge, Skills, and Abilities (KSAs)/ Management Preferences are not required. Applicants who possess the following skills are preferred:

  • Documented experience coordinating multidisciplinary evaluations and assessments for infants and toddlers, including scheduling, collecting developmental data, and integrating findings across required domains.

  • Proven experience developing and implementing Individualized Family Service Plans (IFSPs) in collaboration with families and service providers, while meeting federally mandated timelines and documentation standards.

  • Demonstrated experience monitoring early intervention service delivery through review of provider notes, direct observation, and family input to ensure quality, compliance, and progress toward IFSP outcomes.

  • Hands-on experience using electronic data systems to maintain accurate and timely child records, including service authorizations, progress updates, and required reporting for program compliance.

  • Experience facilitating transitions for children exiting early intervention services, including coordinating with receiving agencies, conducting planning meetings, and preparing families for next steps.
