Position

  • Orient new patients to unit/programs.
  • Monitor and facilitate patient activities and adherence to rules and regulations of the agency.
  • Assure delivery of special diet orders to kitchen.
  • Coordinate external patient discharges with nursing and counseling staff.
  • Coordinate patient referrals assuring that patient has all the necessary paperwork and arranges for transportation to referrals.
  • Complete unit filing and telephone back-up as other duties allow.
  • Assist with washing all unit patient blankets and clothing of medically ill patients.
  • Maintain cleanliness of units.

Ensures visitors are approved/ authorized to enter the facility and verifies their departure at the appropriate time such as Peer Mentors and meeting facilitators.

Second shift: Off every other weekend and off every other Monday and Friday.

Duties and responsibilities

  • Patient Care
  • Promote safety above all care
  • Monitor and facilitate patient activities and adherence to rules and regulations of the agency, by conducting regular resident rounds.
  • Perform random Urine Drug Screens, Pregnancy Tests and Breathalyzers.
  • Perform duties to ensure a successful discharge under the coordination of the medical/clinical team.
  • Provide random and or routine resident searches according to procedure
  • Administration
  • Complete admission procedures/documents as assigned.
  • Complete unity filing and telephone back-up as other duties allow.
  • Demonstrates responsibility/caution when using access points to residential and administrative areas.
  • Housekeeping
  • Assist with washing reusable unit linen and clothing of Withdrawal Management.
  • Maintain cleanliness of Withdrawal Management areas thoroughly, and total unit in general, by completing spot cleaning during the day shifts and thorough
    cleaning on night shifts.

Physical requirements

  • While performing the duties of this job, the employee may need to remain in a stationary position for long periods of time or may need to move about for extended periods of time.
  • Position may require employee to have full range of motion and lift up to 30 lbs.
  • Position may require individuals to communicate, converse and exchange information with others. May require individual to communication via email, phone or in person.

Qualifications

  • High school diploma
  • Experience providing customer to the public required.
  • Previous experience in residential or health care setting providing patient monitoring preferred.
