Job Summary
We are seeking a dynamic and compassionate Caregiver Lead to join our dedicated team in providing exceptional support and care to individuals with diverse needs. Duties

• Act as a backup caregiver during peak times or when additional support is required.
• Responsible for ensuring the continuity of care in the patients' homes utilizing the appropriate resources and office staff.
• Report findings to the Care Giver Supervisor and / or office staff in a timely manner.
• Consistently engages in huddles and interacts with all office staff to ensure that we have the appropriate number of caregivers to meet the clients' needs.
• Consistently engages in huddles and interacts with all office staff to ensure that we are meeting the needs of all our patients.
• Demonstrates accountability and ownership of patient care.
• Seeks and identifies opportunities for process improvement.
• Expected to participate in weekend and holiday coverage rotation per business needs.
• Meets or exceeds productivity targets.
• Performs additional duties as assigned.
• Perform personal care tasks such as bathing, grooming, and toileting.
• Assist with activities of daily living, including meal preparation, medication reminders, and transportation.
• Communicate regularly with clients, their families, and team members to ensure their needs are met and concerns are addressed.
• Train and mentor caregivers, providing guidance, support, and constructive feedback to improve their performance
• Conduct visits to ensure the quality of care being provided meets company standards
• Maintain accurate and thorough documentation of all client care and caregiver activities.
• Adhere to all company policies, procedures, and safety guidelines

Qualification

• Current California Health Care Aid (HCA) Certification
• 6-10 years of caregiver experience
• Knowledge of current standards of patient care for an HCA
• The ability to handle the concerns of clients and caregivers and knowing when to elevate the concerns to the Caregiver Supervisor and / or the office staff.
• The ability to monitor and document care issues or concerns.
• Computer literate
• Ability to make sound, independent judgements, and act professionally under pressure.
• Demonstrates effective documentation and verbal communication skills: factual, accurate, clear, and timely to support delivery of effective patient care and collaborative management.
• Knowledge and understanding of caregiving principles and techniques.
