Pay Range: $29 – $31 per hour

We are seeking a Licensed Practical Nurse (LPN) to join our supportive and mission-driven team. The LPN will play a vital role in providing high-quality clinical care, supporting daily health needs, and fostering a safe and positive environment for the individuals we serve.

• Provide skilled nursing care in accordance with state regulations and company policies.

• Administer medications and treatments as prescribed.

• Monitor and record vital signs, symptoms, and patient progress.

• Collaborate with physicians, RNs, and caregivers to ensure continuity of care.

• Educate patients and families on health management and wellness.

• Maintain accurate, timely, and confidential medical records.

• Respond to medical emergencies with professionalism and urgency.

• Active Licensed Practical Nurse (LPN) license in the state of (Insert State).

• CPR and First Aid certification (or willingness to obtain).

• Strong clinical and critical thinking skills.

• Compassionate, reliable, and dedicated to patient care.

• Excellent communication and teamwork abilities.

• Experience in home health, community health, or long-term care is preferred but not required.
