Job Summary:

Must have an understanding of home community based services for the mentally and/or physically handicapped. Must have the ability to develop home community based services in accordance with Federal, State & local regulations. Must have a basic understanding of behavior problems of the mentally handicapped. Ability to supervise employees and consumer. Ingenuity in the preparation and scheduling of HCBS. Ability to work independently and to be flexible when conflicts arise. Ability to follow oral or written directions and instructions. Ability to supervise and assist staff and person served in their home setting.

Job Responsibilities:

  • Efficient and effective operation of New Hope staff in conformity with the ideals of the established objectives of the programs according to the HCBS waiver for the MR/DD individual.
  • Adherence to company policies and procedures, standards of conduct and maintain a professional attitude at all times.
  • Assist with and/or perform procedures as outlined with proper instruction and supervision.
  • Observe consumers physical condition and report any significant changes to consumer's health care provider and/or notify the on-call person.
  • Observe consumers mental status, behavioral responses to day-to-day activity and to report any significant changes to the on call person.
  • Maintain and review consumer documentation (MARs, Budget sheets, Shopping needs) on a regular basis.
  • Implement and supervise to ensure documentation is completed correctly and accurately.
  • In coordination with the Program Manager complete disciplinary actions with staff as needed.
  • Attend in-service meetings as required.
  • Assisting with residential and day service activities or duties as needed.
  • Will participate in referral, planing, initiating, and evaluation monitoring of programs as directed by supervisor, and assisting consumers in their pursuit of independence.
  • Perform training functions as assigned and implement training programs as directed.
  • Will have knowledge, understanding and ability to carry out the behavior management Support Plan. Will conduct special behavior management programs as directed. Must know and understand all regulations, such as Article 63, Person Centered Support Planning and Kansas Lifestyle Outcomes.
  • Attend interdisciplinary team meetings as directed by Program Director.
  • Report and record all unusual happenings with pertinent information to the appropriate person, supervisor, healthcare provider and/or guardian.
  • On call rotation
  • Maintain current and valid driver's license of the state of residence and be determined as an "insurable driver" by the automobile insurance carrier for New Hope unless other arrangements can be made. And provide insurance documentation on private vehicles of liability coverage insurance.
  • Will comply with all federal, state and local laws regarding communicable and infectious diseases.
  • Have NO records of conviction of abuse, neglect or exploitation of any individual minor or adult. Be willing to undergo a KBI check, not be convicted of any crimes against persons, sex crimes, crimes against property, violations of personal rights, public safety, public morals, or any crimes that would be classified in Kansas as a felony or is a crime of moral turpitude or dishonesty.
  • Read and comprehend written materials generated by New Hope including policies and procedures and standard of conduct handbook.
  • Other duties as assigned.
  • Must be flexible with hours to meet Agency needs
  • Will adhere to the HCBS dress code as signed

Experience:

  • High school graduate or G.E.D. certificate preferred.
  • One year in providing services to the intellectually and developmentally disabled preferred.
