About Forward Leading IPA (FLIPA)
Forward Leading IPA (FLIPA) is a nonprofit membership association of safety‑net providers dedicated to delivering high‑quality, integrated healthcare to historically underserved communities across Upstate New York. Since 2017, FLIPA has been recognized for its commitment to integrating primary care, behavioral health, and social care. Our membership includes federally qualified health centers (FQHCs), behavioral health organizations, and a rural health network representing eight county public health departments.

As part of the statewide Social Care Network (SCN), FLIPA is helping build a more equitable, connected, and person‑centered system for Medicaid members. We're looking for a Community Health Worker Supervisor who is passionate about supporting frontline staff, strengthening community partnerships, and driving meaningful impact.
What You'll Do
- Lead, coach, and support a team of Community Health Workers (CHWs) embedded at FLIPA Member Centers.
- Ensure CHWs deliver culturally responsive, person‑centered screening, navigation, and care management services.
- Build a collaborative learning environment that encourages growth and continuous improvement.
- Ensure CHWs complete all required SCN, FLIPA, and Center‑specific trainings.
- Monitor performance, track progress toward goals, and elevate successes and challenges to FLIPA leadership.
- Partner closely with on‑site CHW supervisors to support scheduling, communication, and problem‑solving.
- Participate in regular check‑ins with Member Centers to strengthen relationships and alignment.
- Perform additional duties as assigned.
Qualifications
- Bachelor's degree in human services or related field preferred; equivalent experience considered.
- Minimum of 1–3 years of Supervisory experience.
- Strong communication, relationship‑building, and customer service skills.
- Ability to manage multiple priorities, maintain composure under pressure, and adapt to changing needs.
- Proficiency with basic computer applications, including Microsoft Office.
