RN Case Manager

**Overview:**

We are seeking a dedicated Registered Nurse Case Manager to join our team. This position involves providing comprehensive patient care through case management and coordination of services.

**Duties:**

• Conduct patient assessments and develop individualized care plans

• Coordinate discharge planning and ensure smooth transitions for patients

• Collaborate with healthcare providers to optimize patient outcomes

• Provide critical care support and interventions as needed

• Utilize behavioral health knowledge to address patient needs

• Manage patient services efficiently and effectively

**Qualifications:**

• Valid RN license

• Previous experience in case management preferred

• Strong understanding of patient care and physiology

• Knowledge of hospital procedures and protocols

• Excellent communication and interpersonal skills

**Skills:**

• In-depth knowledge of patient care and critical care practices

• Proficiency in conducting patient assessments and developing care plans

• Experience in discharge planning and coordination of services

• Strong understanding of hospital operations and procedures

• Ability to provide exceptional patient service

• Home Health Experience
