Community Health Worker

Position Overview: We are seeking a passionate and dedicated Community Health Worker (CHW) to join our team. The CHW will play a vital role in connecting patients to healthcare services, providing education on health issues, and advocating for community health initiatives. This position requires strong communication skills, cultural competence, and a commitment to improving health outcomes for our community.

Key Responsibilities: Outreach and Engagement: Build relationships with community members to promote health services and initiatives, particularly among underserved populations. Education and Advocacy: Provide information and resources on health topics, including preventive care, chronic disease management, and available community resources. Care Coordination: Assist patients in navigating the healthcare system, including scheduling appointments, accessing services, and following up on care plans. Data Collection: Gather and maintain accurate data on community health trends and patient needs to inform program development and improvement. Collaboration: Work with healthcare providers, community organizations, and stakeholders to develop and implement health programs and initiatives. Support Services: Offer emotional support and guidance to patients and families, addressing barriers to accessing care.

Qualifications:

• Must be Bilingual (English/Spanish) High school diploma or equivalent; associate's or bachelor's degree in public health, social work, or a related field preferred.
• Must have a Community Health Worker (CHW) certificate
• Previous experience as a Community Health Worker or in a similar role
• Knowledge of local health resources and community services.
• Strong interpersonal and communication skills, with the ability to engage diverse populations.
• Ability to work independently and as part of a team.
