Gateway Recovery Center is a dual diagnosis, DCF & Joint Commission accredited facility found in Margate, Florida hat provides a Intensive Outpatient Treatment Program. Our lives don't stop while we seek treatment for substance abuse. An IOP (intensive outpatient program) is the most flexible option for rehabilitation. Our program gives you access to a structured environment with treatment from leading industry professionals, all while offering our clients the flexibility to continue going about their everyday lives while getting the help they need to achieve purposeful, long-term sobriety.
Qualifications:
- Ability to work in our clinical office between the hours of 5PM and 9PM Monday through Friday. We do NOT have a daytime program or track.
- Preferred experience: 1-2 years in recovery-related position/field.
- Familiarity with day-to-day operations at IOP & OP LOC.
- Knowledgeable about all stages of treatment, relapse, and recovery.
- High School Diploma or GED required.
- Must be CPR, AED & First Aid Certified after first 30 days.
- Must not have any record that contains any of the listed "Disqualifying Offenses" in the DCF & FARR Affidavit of Good Moral Character. For a list of these offenses, refer to:
Position Summary: The Case Manager is responsible for the coordination of care and health management for our clients. Core functions include assessing clients' needs upon intake and throughout the period of treatment, primarily focused on:
· Assistance with intakes and clinical assessments specific to admissions and monitoring client progress throughout the stay from the Pre-Admission phase through Discharge.
· Assistance and communication regarding legal needs, support, and communication with probation officers, attorneys, courts, etc.
· Assistance with job searches, resume building, schooling/further education enrollment applications.
· Discharge assistance and planning regarding aftercare and long-term living arrangements – sober living, returning home, continued psychiatric care, medication management, etc.
· Assist clients in obtaining Government Identification, Birth Certificate, Social Security Card and Disability Benefits as applicable, as well as food stamps.
· Assisting the medical team in the arrangement of medical care and appointments with specialists to address specific medical issues and/or concerns. (Including OB/GYN, dentists, neurologists, gastroenterologists, cardiologists, etc.)
· Documentation of weekly case management progress in KIPU.
· Acting as a liaison between clients and the treatment team as needed.
· Assisting the medical team in the arrangement of medical care and appointments with specialists to address specific medical issues and/or concerns. (Including OB/GYN, dentists, neurologists, gastroenterologists, cardiologists, etc.)
· Assistance with the completion and submission of FMLA & short-term disability, along with coordination of benefits when necessary with insurance payers.
· Follow company policy and procedures and understand & follow all State and Federal HIPAA laws pertaining to confidentiality.
· Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with organizational policies.
