Community Health Worker (CHW)

Full-Time | Community-Based Role

Do you have a heart for helping others and a strong connection to your community? We're looking for a Community Health Worker (CHW) to join our care coordination team and support patients where it matters most — in their homes and communities.

As a CHW, you'll work directly with high-risk patients, providing education, encouragement, and hands-on support to help them manage chronic conditions, access resources, and stay connected to their healthcare team.

What You'll Do

• Visit patients in their homes and community settings to provide ongoing support
• Help patients understand and follow care and self-management plans
• Provide social, emotional, and practical support as patient needs change
• Serve as a liaison between patients, providers, and community resources
• Assist with care coordination, health education, and access to services

What We're Looking For

• A trusted, compassionate member of the community
• Strong communication and interpersonal skills
• Able to understand written materials and document information clearly
• Ability to show empathy and build meaningful relationships
• Knowledge of local community resources
• Valid, unrestricted driver's license in the state of employment
• Position will require frequent travel.
