Responsibilities:

  • Create and implement personalized treatment plans for home-based pediatric and adult therapy patients
  • Conduct initial and ongoing comprehensive assessments of the patient's needs
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and families
  • Maintain accurate and timely documentation of patient progress and treatment plans to follow Colorado Home Health guidelines
  • Demonstrate a strong foundational knowledge of speech-language pathology and the ability to effectively problem-solve in diverse situations
  • Track hours and maintain ASHA compliance for completion of CFY year

Locations of Need:

  • North Aurora (Green Valley Ranch, Commerce City)
  • South Aurora/Centennial/Parker
  • North Denver, Westminster, Thornton, Wheat Ridge, Northglenn, Arvada
  • Central Denver, Glendale
  • Castle Rock (*Maternity Leave Coverage needed starting January 2026)

Pay Structure:

  • $55 per regular visit minute visits at your clinical discretion)
  • Start of Care Evaluations- $75/visit
  • Recertifications/Re-Evaluations (Every 60 days)- $60/visit

Requirements:

JOB CONDITIONS

  • The ability to drive and the ability to access consumers' homes which may not be routinely wheelchair accessible are required.
  • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the consumer's condition and to perform consumer care.
  • The ability to communicate well, both verbally and in writing in English is required

EQUIPMENT OPERATION

Speech therapy equipment and hand washing materials.

COMPANY INFORMATION

Has access to all consumer medical records which may be discussed with Nursing Supervisor, the Administrator, the Executive Director, and the Quality Assurance department staff and government agencies

QUALIFICATIONS

  • Must hold a Master's degree in Speech Therapy from an accredited program
  • Licensed to practice Speech Therapy in the state of Colorado
  • CPR certification
