Job Summary
The Activities Coordinator will design and lead therapeutic recreation programs for adults and seniors, with a strong emphasis on dementia-friendly programming. This role requires prior experience in activity planning and coordination in senior care, or adult day setting. This role focuses on enhancing the quality of life for seniors, particularly those with memory care needs, including Alzheimer's and dementia. The Activities Coordinator will work closely with residents, families, and staff to create a vibrant community atmosphere that promotes social interaction, physical activity, and emotional well-being.
Duties
- Develop and implement a comprehensive activities program tailored to the interests and abilities of residents.
- Plan and coordinate events, outings, and recreational activities that encourage participation from all residents
- Proven experience in event planning within assisted living or long-term care environments.
- Knowledge of Alzheimer's care, dementia care, memory care, and recreational therapy principles.
- Strong background in senior care, caregiving, or home & community care experience.
- Excellent communication skills to interact effectively with residents, families, and team members.
- Ability to manage schedules efficiently while accommodating the diverse needs of residents.
- Experience working with individuals with disabilities is highly desirable.
- Understanding of aging issues counseling and social work principles is a plus. This position requires a compassionate individual dedicated to enriching the lives of seniors through meaningful activities. The ideal candidate will possess a blend of creativity, organizational skills, and a genuine passion for working with older adults.
- Participate as a member of the Interdisciplinary Care Team (IDT). As a contributing member of the IDT, attends all relevant meetings, prepares in advance for contribution and information sharing, completes follow-up documents, and provides status reports to other team members.
- Perform home visits to PACE participants as needed for participant assessment and care planning.
- Coordinates between other operational areas that include but are not limited to transportation, nutrition services, social services, nursing, and rehabilitation.
- Completes timely documentation including progress notes, assessments, and reassessments as well as other data as needed.
- Coordinate group and individual activities and activity planning that address the participant's cognitive, physical, social, psychosocial, spiritual, and artistic needs.
- Maintain inventory of recreational supplies and equipment. Ensure equipment is maintained and in good working condition. Initiate orders for repair or replacement as required.
- Prepare a monthly recreation calendar for the DHC
- Schedule and facilitate trips to destinations of interest to the participants. Arrange for any prior approval of expenditures with the Center Manager.
- Decorate the PACE Center for seasonal and holiday themes.
- Encourage participants to participate actively in programs.
- Review participant clinical information in medical records on admission, with semiannual reassessments, and reassessments for significant change of condition.
- Complete initial, semi-annual, and significant change of condition. Develop a recreation plan of care to address the physical and cognitive levels of participants as well as their leisure and cultural preferences and share with IDT.
- Other duties as assigned.
Required Education and Experience:
- A bachelor's degree in aligned field is acceptable with appropriate work experience.
- Certified Therapeutic Recreation Specialist (CTRS) or Recreation Therapy license
- Minimum of two (2) years of general clinical experience in home care, nursing home, acute care, or long-term care setting, supporting aging adults with functional impairments.
- A minimum of one (1) year of Activities Coordinator or Therapeutic Recreation experience.
