Job Summary
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to provide high-quality patient care in private residences of clients. The ideal candidate will have experience in diverse clinical environments, including long-term care, acute care, and home health, or memory care. This role involves working closely with patients of all ages and backgrounds, ensuring their comfort, safety, and well-being while adhering to healthcare standards and regulations. The CNA will play a vital role in supporting medical teams by assisting with daily patient needs and documentation within electronic health record systems.
Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature
- Support medication administration under supervision, ensuring proper documentation in EMR/EHR systems
- Provide specialized care for patients with dementia, Alzheimer's disease, or memory impairments, including behavioral health management
- Maintain infection control protocols using aseptic techniques and adhere to HIPAA regulations to protect patient confidentiality
- Assist with catheter care, wound dressing changes, and other medical procedures as directed by nursing staff
- Support patients in rehabilitation or post-acute care settings by assisting with physiologic needs and mobility exercises
- Document patient observations and care activities thoroughly in medical records
- Collaborate with interdisciplinary teams in hospital or outpatient settings such as clinics or urgent care centers to ensure comprehensive patient care
Skills
- Proven experience in various healthcare environments including hospital medicine, long-term care, assisted living, hospice care, or home health
- Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks for accurate documentation
- Knowledge of medical terminology, anatomy, physiology, and infection control practices
- Ability to work effectively with diverse patient populations including geriatrics, individuals with disabilities or developmental disabilities
- Experience with specialized care such as Alzheimer's/dementia care
- Competence in basic life support (BLS) certification and patient service skills
- Strong communication skills to collaborate effectively within healthcare teams and provide compassionate patient support
- Ability to maintain professionalism under pressure while adhering to safety standards and confidentiality regulations
