Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Monitor for clients safety, observe and report any changes in condition or behavior.
• Support medication reminders, following care plans and HIPAA confidentiality standards to ensure safety and privacy.
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
• Maintain a clean and safe environment by performing light cleaning duties and ensuring proper sanitation practices are followed.
• Document care activities accurately in patient records and update supervisor changes needed in care plans regularly to reflect evolving needs.

Skills

• Proven experience in home care, HHA, assisted living, nursing homes, or group homes is highly desirable.
• Knowledge of patient observation techniques and ability to monitor vital signs effectively.
• Certification in CPR and First Aid to respond swiftly during emergencies.
• Familiarity with dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism.
• Ability to perform heavy lifting safely and assist residents with mobility challenges
• Strong communication skills for collaborating with healthcare teams, social workers, care coordinators and family members while maintaining HIPAA compliance.
