Purpose
To provide client-centered navigation and coordination of behavioral health services, ensuring timely access to care, continuity of treatment, and integration with Sanctuary Recovery Centers' programs.
Scope
This position applies to all behavioral health navigation activities, including client engagement, care coordination, and linkage to community resources.
Responsibilities
Client Engagement & Assessment
- Serve as the primary point of contact for clients entering behavioral health services.
- Conduct initial screenings and assist with intake documentation.
Care Coordination
- Develop individualized navigation plans in collaboration with clinical staff.
- Schedule appointments, follow up on referrals, and ensure continuity of care.
Resource Linkage
- Connect clients to internal programs and external community resources for housing, employment, and social support.
- Maintain updated resource lists and referral pathways.
Compliance & Documentation
- Document all client interactions in accordance with Sanctuary Recovery Centers' policies and state regulations.
- Participate in monthly clinical oversight meetings as required under P&P guidelines.
Qualifications
- Minimum of one year experience in behavioral health or social services; preference for candidates with relevant degree.
- Must be 21 years of age and able to provide proof of identification.
- CPR and First Aid certification; AZ Food Handlers Card if applicable.
Competencies
- Strong communication and interpersonal skills.
- Ability to work collaboratively with clinical teams and community partners.
- Knowledge of behavioral health systems and recovery-oriented care.
Physical Requirements
- Ability to sit for extended periods and work on a computer.
- Occasional travel to community partner sites.
