Join our dynamic healthcare team as a Nurse Practitioner specializing in Geriatrics within Skilled Nursing Facilities (SNF) and Long-Term Care (LTC) settings. Duties
- Conduct thorough physical examinations and health assessments for geriatric patients in SNF/LTC environments
- Develop, implement, and adjust individualized care plans based on patient needs and clinical findings
- Manage acute and chronic conditions, including medication management, IV infusions, and pain control strategies
- Perform procedures such as suturing, catheterization, spinal taps, and dermal filler injections when appropriate
- Coordinate with interdisciplinary teams on discharge planning, case management, and utilization review to ensure seamless patient care transitions
- Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently
- Engage in telehealth consultations to extend care access and support remote patient management
- Provide education on health coaching topics such as nutrition, medication adherence, dementia care, and hospice & palliative medicine
Experience
- Proven experience as a Nurse Practitioner with a focus on geriatrics or senior care in SNF/LTC settings
- Demonstrated proficiency with EMR/EHR systems including Epic, Athenahealth, Cerner or similar platforms
- Experience performing advanced procedures such as IV insertions, venipuncture, sterile processing techniques, and basic life support (BLS) certification
- Background working with diverse populations including elderly residents with dementia or Alzheimer's disease; familiarity with memory care is a plus
- Knowledge of CPT coding, ICD-10/ICD-9 coding for accurate documentation and billing processes
- Critical care experience in ICU or emergency room environments is highly desirable
- Familiarity with infection control protocols, infection disease care, acute pain management, and hospital medicine practices
- Ability to triage urgent cases effectively while managing multiple priorities in fast-paced settings such as assisted living or post-acute care facilities
- Strong understanding of Workers' Compensation law and utilization review procedures for comprehensive patient management
