Case Management Specialist (Shelter Part-Time)

Position Summary

Under the supervision of the Programs Manager, the Case Management Specialist provides direct case management, peer support, and coordination of care for youth enrolled in the Runaway and Homeless Youth (RHY) Shelter and Transitional Living Program (TLP). The Case Management Specialist ensures the delivery of trauma-informed, culturally responsive, and strengths-based services that promote youth stabilization, well-being, and long-term self-sufficiency.

This position serves as the senior peer-support and case management role within the program, supervising case planning processes for both Peer Counselor 1 and Peer Counselor 2 positions and collaborating closely with the Mental Health and Counseling team to support client treatment plans and service coordination.

Essential Duties and Responsibilities

· Manage a caseload of youth clients across Shelter and TLP programs, providing individualized case management and peer support.

· Conduct comprehensive intake and assessment to identify each client's needs, strengths, and goals.

· Develop, implement, and monitor individualized case plans in collaboration with clients, Peer Counselors, and multidisciplinary staff.

· Provide ongoing coaching, motivation, and crisis intervention to help youth progress toward educational, vocational, behavioral health, and housing goals.

· Collaborate with the Mental Health/Counseling team to execute components of treatment plans, coordinate services, and ensure continuity of care.

· Maintain accurate, timely documentation of all client interactions, case notes, and service outcomes within the Electronic Records Management (ERM) system.

· Assist clients in accessing mainstream benefits and community supports, including Medicaid/OHP, SNAP, employment services, and education programs.

· Conduct follow-up and aftercare services upon program exit to promote long-term stability.

· Work in alignment with FYSB RHY program standards, ensuring case management practices meet all grant and licensing requirements.

· Support billing and service authorization processes by verifying documentation accuracy, tracking service hours, and communicating with administrative staff.

· Prepare reports, track outcomes, and assist with required grant documentation.

· Provide leadership, mentoring, and case coordination guidance to Peer Counselors 1 and 2.

· Assist with quality assurance, program evaluations, and outcome tracking for RHY and TLP programs.

· Build and maintain relationships with external partners, including schools, employers, health providers, and housing programs.

· Advocate for youth in accessing appropriate community-based services.

· Represent Integral Youth Services professionally in community coalitions, committees, and partner meetings.

Qualifications

· High school diploma or GED.

· Associate or bachelor's degree in human services, Social Work, Psychology, or related field preferred.

· Certified Peer Support Specialist for both Youth and Adult Mental Health.

· Registered Traditional Health Worker (THW) with an active NPI number.

· Minimum of 3 years of experience in case management or youth services, or a combination of education and relevant fieldwork.

· Demonstrated experience with trauma-informed care, positive youth development, and harm reduction principles.

· Strong written and verbal communication skills, with the ability to maintain detailed and compliant documentation.

· Proficiency with electronic records and case management systems.

· Valid Oregon driver's license and insurable driving record.

· Ability to pass DHS background and abuse registry checks.

. Ability to pass a drug screening.

· Current First Aid/CPR certification (or ability to obtain within 30 days of hire).

Preferred:

· Experience with RHY, FYSB, or federally funded youth programs.

· Supervisory or mentoring experience.

· Familiarity with Medicaid billing and service documentation standards.

Job Type: Part-time

Pay: $21.00 per hour
