WV Primary Care is a patient oriented Primary Care Medical Office located at 924 Cross Lanes Dr, Cross Lanes. We are looking to hire a Nurse Practitioner or Physician Assistant either full or Part-time position. Friendly and Co-operative Co-workers. Positive Work Environment. Schedule: Variable between 7:30 AM to 7 PM MAXIMUM 42.5 hrs/Week ( Including Lunch)

Reports To: Practice Physician and Office Manager

Bonus: Based on the Number of Patients evaluated after one year. Negotiable.

Minimum Qualification: Education, Experience, Licensure:
For Nurse Practitioner: A Master's Degree in Nursing with certification as an advanced practice registered nurse is required. Licensure, or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia, requires national Certification by one of the recognized APRN Certification Centers
For Physician Assistant: Bachelor's Degree as a Medical/Surgical (as applicable) Physician Assistant is the minimum degree required. Masters level education as Medical/Surgical (as applicable) Physician Assistant preferred. A graduate of an American Medical Association (AMA) approved Physician Assistant (PA) training program required. Certification by the National Commission for Certification of Physician Assistants is required. Must be licensed or eligible for licensure by the WV Board of Medicine.

Summary: The Advance Practice Professional (APP) is an individual with expertise in the management of the medical patients throughout their lifespan, with a focus on both acute and chronic condition management for the patient to maintain optimum health. The APP completes comprehensive patient health evaluations (including biological, social, and psychological assessments) with the appropriate clinical/faculty team. The APP assesses and coordinates the educational and psychosocial needs of the patient and caretakers with a focus on acute issues, maximizing the stability of chronic conditions, disease prevention, and overall medical management of the patient. The APP develops and reviews the plan of care with the other members of the medicine clinical team, participates in directing that care, and coordinates planning for patient follow-up across the continuum of care (as applicable).

Manages comprehensive and individualized care for patients and their families in the clinic. Completes pre-procedure evaluations. Performs, interprets, and communicates results of diagnostic tests in the outpatient setting. Consult and communicate with clinic staff regarding abnormal findings in the evaluations of patients. Performs history and physical examinations and documents findings in the medical record. Facilitates patient follow-up maintenance by coordinating appropriate services and tests prior to scheduled outpatient follow-up appointments in the clinic (as applicable). Performs necessary procedures following established clinical guidelines and Protocols and scope of practice (SOP).
