Responsibilities
- Conduct comprehensive patient assessments to determine physical therapy needs, utilizing your expertise in anatomy and physiology.
- Develop and implement tailored care plans that address acute conditions, orthopedic injuries, geriatrics, pediatrics, and other specialized areas.
- Provide hands-on therapy techniques including trigger point therapy and other manual interventions to alleviate pain and improve function.
- Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes.
- Educate patients and their families on injury prevention, rehabilitation exercises, and home care routines to support ongoing recovery.
- Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital to home or community care settings.
- Document all patient interactions accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
- Collaborate with healthcare professionals across inpatient, outpatient, clinic, and home health environments to deliver comprehensive patient care.
- Participate in care plan meetings and contribute insights on patient progress and treatment efficacy.
- Stay current with advancements in physiotherapy techniques, including trigger point therapy and specialized treatments for orthopedics and geriatrics.
Skills
- Strong knowledge of anatomy, physiology, and medical terminology essential for accurate assessment and treatment planning.
- Proven experience in acute care settings, hospital environments, outpatient clinics, or home health services.
- Expertise in developing effective care plans tailored to diverse populations such as pediatrics and geriatrics.
- Proficiency with EMR systems for documentation and patient management.
- Excellent patient care skills with the ability to communicate clearly and empathetically with patients of all ages.
- Familiarity with discharge planning processes to facilitate smooth transitions from hospital to community or home settings.
- Hands-on experience with physiotherapy techniques including trigger point therapy for pain relief.
- Knowledge of orthopedics and musculoskeletal rehabilitation strategies.
- Experience working within inpatient or outpatient settings with a focus on patient-centered outcomes.
- Ability to work collaboratively within multidisciplinary teams across various healthcare environments.
- Home & community care experience is a plus for supporting patients outside clinical settings. Pay: $85, $100,000.00 per year
