Responsibilities

• Conduct comprehensive patient assessments to determine physical therapy needs, utilizing your expertise in anatomy and physiology.
• Develop and implement tailored care plans that address acute conditions, orthopedic injuries, geriatrics, pediatrics, and other specialized areas.
• Provide hands-on therapy techniques including trigger point therapy and other manual interventions to alleviate pain and improve function.
• Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes.
• Educate patients and their families on injury prevention, rehabilitation exercises, and home care routines to support ongoing recovery.
• Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital to home or community care settings.
• Document all patient interactions accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
• Collaborate with healthcare professionals across inpatient, outpatient, clinic, and home health environments to deliver comprehensive patient care.
• Participate in care plan meetings and contribute insights on patient progress and treatment efficacy.
• Stay current with advancements in physiotherapy techniques, including trigger point therapy and specialized treatments for orthopedics and geriatrics.

Skills

• Strong knowledge of anatomy, physiology, and medical terminology essential for accurate assessment and treatment planning.
• Proven experience in acute care settings, hospital environments, outpatient clinics, or home health services.
• Expertise in developing effective care plans tailored to diverse populations such as pediatrics and geriatrics.
• Proficiency with EMR systems for documentation and patient management.
• Excellent patient care skills with the ability to communicate clearly and empathetically with patients of all ages.
• Familiarity with discharge planning processes to facilitate smooth transitions from hospital to community or home settings.
• Hands-on experience with physiotherapy techniques including trigger point therapy for pain relief.
• Knowledge of orthopedics and musculoskeletal rehabilitation strategies.
• Experience working within inpatient or outpatient settings with a focus on patient-centered outcomes.
• Ability to work collaboratively within multidisciplinary teams across various healthcare environments.
• Home & community care experience is a plus for supporting patients outside clinical settings. Pay: $85, $100,000.00 per year
