Position Overview:
The Specialized Transitional Home is a program designed to assist three to four individuals whose needs cannot be met by less intensive support options. This program offers support in learning life and leisure skills, personal care, and community integration, as outlined in each person's individualized plan
Location/Schedule – 1881 Twin Pines Drive, Macon, Georgia Rotating shifts Sun-Sat 7am-7pm or 7pm-7am
Responsibilities:
- Administer medications as prescribed by healthcare providers to patients in the home.
- Regularly assess and record patients' vital signs to monitor stability and identify any changes that may indicate a decline in condition.
- Assist individuals with daily personal care needs in the home.
- Treat and dress wounds, injuries, and other physical health concerns that may arise during a crisis.
- Assist in managing and de-escalating crises, utilizing clinical knowledge and training to support patients in distress.
- Work closely with the BCBA and other healthcare professionals—including physicians, psychologists, social workers, and nursing staff—to provide coordinated and comprehensive care.
- Maintain accurate, up-to-date patient records, including assessments, interventions, treatments, and outcomes, in accordance with legal and ethical documentation standards.
- Follow all safety protocols and procedures to ensure a safe environment for both patients and staff within the home.
- Perform housekeeping duties such as washing clothes/dishes, dusting, running errands, wiping baseboards, sweeping, mopping, and related tasks as needed.
Qualifications Needed (education and/or experience):
- Current Georgia license to practice as a licensed practical nurse.
- At least 1 year working in behavioral health/ intellectual and developmental disabilities (IDD) or related field
- Must have good computer skills.
- Communications skills and the ability to foster good relationships among others is a plus.
