Discharge Planner

* This position is located in the Monterey Correctional Facilities. 1 PT and 1 FT position available - 40hrs/wk & 20hrs/wk

Job Summary
As a Case Manager/Discharge Planner, you will play a crucial role in coordinating patient care services within a correctional setting, particularly focusing on the juvenile population transitioning back into the community. Your responsibilities will involve utilizing your knowledge of medical and mental health needs and services to ensure accurate documentation and efficient patient service.

Responsibilities
- Collaborate with healthcare professionals to develop and implement patient care plans
- Maintain accurate and up-to-date documentation related to patient care
- Coordinate appointments, treatments, and consultations for patients
- Provide support to families by providing education/psychoeducation

Qualifications
- Experience in discharge planning and case management

• Ability to work in acute care settings and develop care plans

• Knowledge of medical and mental health records management and documentation

• Familiarity with medical and mental health terminology and procedures; proficiency in HIPAA regulations

• Ability to work collaboratively with a multidisciplinary team

• Excellent communication and interpersonal skills for effective patient interaction

• Knowledge of corrections facility medical and mental health clinic operations preferred, but not required

-Strong background in patient service within an inpatient or correctional setting preferred, but not required
