McLellan Homecare provides comprehensive speech and language, occupational, and physical therapy services for children from birth to age 18.
We are currently seeking to hire a physical therapist to provide services in the Boulder County area (Boulder, Longmont, Superior, Louisville, Lafayette, and Broomfield).
The physical therapist will provide direct in-home intervention services to children referred and enrolled in Part C Early Intervention services (birth to three years old), as well as children over the age of three. Supervision of some patients seen by a physical therapy aid is also required. They will also provide consultation to Service Coordinators, contracted providers, and families, to assist in meeting the needs of children. Further duties are supervision of a physical therapy assistant, including monthly visits with their patients as well as updated plans of care every 60 days.
Essential Duties:
* Performs intervention activities for children birth to 18 in-home, or other community environments;
* Writes progress notes, updates plans of care, and updates IFSP goals at 6 month and annual reviews;
* Creates strategies based on family priorities that are measurable and meaningful;
* Effectively communicates strategies and techniques that are functional for the child’s and family’s life and are approached in a Primary Service provider model;
* May participate in Transdisciplinary team meetings for Early Intervention caseload;
* Manages schedule to accommodate family schedules and meet service hours set by the IFSP and physician-ordered plan of care;
* Is culturally responsive and competent in working with a diverse population and range of communication delays/disorders
Requirements:
* Current state licensure or the qualifications to obtain one;
* Master’s degree from an accredited university;
* At least three references;
* Takes pride in delivering the highest quality of care;
* Collaboration with the team to deliver consistency and excellence in therapy;
* Valid drivers’ license;
* Proof of motor vehicle insurance;
* Background Check;
* Fulfill CCB training requirements (will provide training options)
* No major violations in the past three years.
Job Type:
* Part-time with ability to increase to Full-time; minimum of 10 visits per week required;

* Salary: competitive per visit rate.
