What You'll Do

  • Provide home health physical therapy services based on physician orders.
  • Develop individualized care plans to improve strength, range of motion, and reduce pain.
  • Educate patients and caregivers on therapy techniques and equipment.
  • Collaborate with a dedicated care team.

This role requires a strong foundation in physiotherapy principles, anatomy, physiology, and medical terminology to develop and implement effective treatment plans tailored to individual patient needs. The Physical Therapist will play a vital role in improving mobility, reducing pain, and enhancing the quality of life for diverse patient populations.

Duties

  • Assess patient conditions through detailed evaluations of mobility, strength, and functional abilities using advanced physiotherapy techniques.
  • Develop personalized care plans incorporating evidence-based interventions such as trigger point therapy, orthopedics, geriatrics, pediatrics, and home & community care strategies.
  • Implement therapeutic exercises and modalities to promote recovery and improve patient care outcomes.
  • Coordinate with healthcare teams to facilitate discharge planning and ensure seamless transition of care across settings.
  • Monitor progress regularly and adjust treatment plans accordingly to meet evolving patient needs.
  • Educate patients and their families on injury prevention, rehabilitation exercises, and health maintenance strategies.
  • Document all patient interactions accurately using EMR systems while maintaining compliance with healthcare regulations.
  • Collaborate with physicians and other healthcare professionals to optimize treatment effectiveness in acute care, inpatient, or outpatient environments.

Requirements

  • Valid licensure as a Physical Therapist in South Carolina.
  • Proven experience in diverse clinical settings such as hospital, outpatient clinics, home health, or rehabilitation centers.
  • Strong knowledge of anatomy, physiology, and medical terminology related to musculoskeletal, neurological, and cardiovascular systems.
  • Experience with developing and managing comprehensive care plans for patients across the lifespan, including geriatrics, pediatrics, and orthopedic cases.
  • Proficiency in using EMR systems for documentation and patient management.
  • Familiarity with specialized therapies such as trigger point therapy and techniques relevant to orthopedics and sports medicine.
  • Excellent communication skills for effective patient education and interdisciplinary collaboration.
  • Ability to work independently or as part of a multidisciplinary team in various settings including outpatient clinics, inpatient units, or home & community care environments.
