Role Summary/Overview

We are seeking a compassionate, experienced Clinical Manager to oversee the nursing and care operations of both our skilled nursing and memory care (Connections) units. The successful candidate will drive excellence in clinical care, compliance, and resident outcomes while leading a multi-disciplinary team. This position is central to ensuring our residents receive high-quality, individualized care in alignment with CommuniCare's mission and Holly Hill's commitment to compassionate, person-centered care.

Key Responsibilities

  • Lead, mentor, and supervise nursing staff (RNs, LPNs, CNAs) across skilled nursing and memory care units.
  • Oversee resident assessments, care planning, documentation, and ongoing monitoring to ensure best practices and regulatory compliance.
  • Ensure effective implementation of the Connections memory care program: person-centered routines, quarterly care conferences, family engagement, and safety protocols.
  • Collaborate with the interdisciplinary team (therapists, social workers, physicians) to design and implement individualized care plans for residents with complex medical or cognitive needs.
  • Manage clinical quality and performance metrics (e.g., infection control, wound care, hospital readmission risk) and lead continuous quality improvement initiatives in line with CommuniCare's standards.
  • Oversee medication management, administration protocols, and proper documentation in electronic medical records.
  • Ensure compliance with all regulatory and accreditation requirements (e.g., CMS, state, HIPAA) and maintain high standards of care across all clinical operations.
  • Lead staff training and continuing education (e.g., dementia care, BLS, infection control, documentation, person-centered care).
  • Drive utilization review, discharge planning, and care coordination to support transitions and continuity of care.
  • Conduct regular audits, quality assurance reviews, and care conferences; report on performance metrics to regional/corporate leadership.
  • Foster a positive, resident-centered culture, ensuring staff engagement, communication, and high morale.

Experience & Qualifications

  • 2–5 years of supervisory or management experience leading nursing staff or overseeing clinical operations.
  • Skilled nursing, long-term care, or post-acute care experience preferred.
  • Active and unencumbered RN license in the state of Maryland.
  • Minimum of 5 years clinical nursing experience, with progressive leadership and management roles in skilled nursing, long-term care, or memory care.
  • Deep knowledge of dementia / Alzheimer's care, ideally with experience in a memory care "Connections"-style model.
  • Strong understanding of skilled nursing clinical operations: wound care, IVs, therapies, infection control, etc.
  • Familiarity with quality and regulatory standards in nursing homes / post-acute care.
  • Experience working with interdisciplinary teams (therapy, social work, medical providers) to deliver holistic care.
  • Excellent communication, leadership, and organizational skills.
  • Proficiency in EMR systems (or willingness to learn CommuniCare's systems).
  • Demonstrated ability to lead quality improvement projects and operational change.
