Job Summary
We are seeking a dedicated and compassionate Substance Abuse Counselor specializing in Partial Hospitalization Program (PHP) services. In this vital role, you will provide comprehensive clinical support to individuals struggling with addiction and co-occurring mental health conditions. Your expertise will guide clients through recovery, utilizing evidence-based therapies and personalized treatment plans. Responsibilities

  • Conduct thorough patient assessments, including diagnostic evaluations and intake interviews, to determine individual treatment needs.
  • Develop and implement tailored treatment plans incorporating psychotherapy, motivational interviewing, cognitive behavioral therapy (CBT), and behavioral health strategies.
  • Facilitate individual and group counseling sessions focused on addiction counseling, mental health counseling, trauma-informed care, and family therapy.
  • Engage in crisis intervention and crisis management to support patients experiencing acute mental health or substance use crises.
  • Maintain accurate medical documentation, including progress notes, treatment plans, and medical records compliant with HIPAA regulations.
  • Collaborate with multidisciplinary teams to coordinate case management, discharge planning, and community care services.
  • Utilize research and data collection methods to monitor patient progress and improve treatment outcomes using ICD coding systems (ICD-9/10) for documentation accuracy.
  • Ensure compliance with all regulatory standards related to behavioral health, medical terminology, CPT coding, and medical records management.

Qualifications

  • Bachelor's degree in social work, psychology, counseling, or a related field; Master's degree preferred.
  • Valid licensure or certification as a substance abuse counselor (e.g., ADT, CADC or equivalent).
  • Proven experience in addiction counseling within PHP settings or similar outpatient programs.
  • Strong knowledge of behavioral therapy techniques such as applied behavior analysis (ABA), cognitive-behavioral therapy (CBT), and CPT coding practices.
  • Familiarity with crisis intervention strategies, trauma-informed care including PTSD care approaches, and family/couple counseling.
  • Experience working in home & community care environments as well as hospital settings is highly desirable.
  • Excellent patient assessment skills coupled with the ability to manage complex cases involving mental health disorders and substance use issues.
  • Knowledge of HIPAA regulations, medical terminology, ICD coding systems, and medical documentation standards.
  • Strong communication skills for effective collaboration with healthcare teams, patients, families, and community resources.
