Job Overview
TRUECARE PARTNERS is seeking compassionate and dependable Caregivers / Direct Support Professionals (DSPs) to provide Home and Community-Based Services to individuals in their homes and throughout the community.

This role focuses on Independent Living Skills (IHS with Training) and community integration. Staff support individuals in building daily living skills, accessing community resources while promoting independence, dignity, and person-centered care.

***This position requires a valid Minnesota drivers license, reliable vehicle and auto insurance***

Duties

(Include but are not limited to)

  • Provide 1:1 or 1:2 support to individuals in home and community settings
  • Daily routines and time management
  • Budgeting and basic money management
  • Household organization and cleanliness
  • Appropriate interpersonal and social skills
  • Transportation to appointments, errands, and activities
  • Participation in recreational, social, or vocational activities
  • Cleaning common areas, laundry, and meal preparation
  • Practicing coping skills
  • Supporting structure and routine
  • Follow each individual's Person-Centered Support Plan
  • Accurately document services and complete EVV
  • Maintain client confidentiality and follow HIPAA and 245D standards
  • Attend supervision meetings and complete required trainings

Skills

  • Experience or interest in working with individuals with: Mental health needs, Developmental or physical disabilities, Substance use recovery backgrounds
  • Strong communication and interpersonal skills
  • Ability to work independently and follow structured support plans
  • Compassionate, patient, and dependable
  • Ability to adapt to different home and community environments
  • Basic technology skills for documentation and EVV use
  • Commitment to professionalism, confidentiality, and reliability

Pay: $ $23.00 per hour

Expected hours: 35 per week
