Overview
We are seeking a compassionate and dedicated Social Worker to join our healthcare team. The ideal candidate will provide comprehensive support to patients and their families across various settings, including hospitals, outpatient clinics, nursing homes, and community-based programs. This role involves conducting assessments, developing care plans, and delivering therapeutic interventions to promote mental health and well-being. Candidates with experience in child & family counseling, behavioral health, hospice care, or working with special populations such as seniors or individuals with developmental disabilities are encouraged to apply. A strong understanding of medical documentation, HIPAA compliance, and clinical research is essential to ensure quality patient care.
Responsibilities
- Conduct thorough patient assessments and diagnostic evaluations to determine individual needs and develop tailored care plans.
- Provide clinical counseling services including individual, group therapy, and family counseling focused on mental health, addiction recovery, or behavioral issues.
- Manage casework by coordinating services within healthcare settings such as hospitals, nursing homes, assisted living facilities, and outpatient clinics.
- Assist patients with intake procedures and facilitate access to community resources for ongoing support.
- Document all patient interactions accurately using EMR systems while adhering to HIPAA regulations and maintaining confidentiality of medical records.
- Engage in crisis intervention and crisis management to support patients experiencing acute mental health or behavioral challenges.
- Collaborate with multidisciplinary teams including physicians, nurses, therapists, and social service providers to ensure comprehensive patient care.
- Participate in clinical research projects related to behavioral health or specific populations such as pediatrics or geriatrics.
- Support special education initiatives or working with children with developmental disabilities or autism spectrum disorder when applicable.
- Maintain up-to-date knowledge of ICD coding (ICD-9/ICD-10) and CPT coding for accurate documentation and billing purposes.
Experience
- Proven experience in social work within healthcare environments such as hospitals, clinics, nursing homes, hospice care, or outpatient settings.
- Demonstrated expertise in child & family counseling, behavioral therapy (including cognitive-behavioral therapy), motivational interviewing, or crisis intervention.
- Familiarity with medical terminology, medical documentation standards, ICD coding systems (ICD-9/ICD-10), and EMR systems is highly desirable.
- Experience working with diverse populations including seniors (geriatrics), individuals with disabilities or developmental disorders (autism spectrum disorder), dementia or Alzheimer's care patients is preferred.
- Background in mental health counseling related to PTSD care or post-acute care is advantageous.
- Knowledge of HIPAA compliance and experience managing sensitive medical records are essential for ensuring privacy standards are met.
