Role Summary
Support two SFM practitioners (combined panel ~300–500 patients) by coordinating pre-visit and post-visit workflows, facilitating communication, and ensuring clinical follow-through. The role may include periodic in-person tasks (e.g., scanning/filing, couriering documents, supporting patient kit logistics, assisting with local events/new-patient group sessions).
Key Responsibilities — Communication, Coordination, Workflow Refinement
· Communicate with clinicians via secure messaging app and work email (on your phone and laptop) to address patient-specific needs, special requests, and care coordination tasks.
· Triage and resolve patient FAQs using internal guidance documents, standardized handouts, and clinic SOPs; escalate appropriately.
· Occasionally facilitate a virtual New-Patient Orientation group (standardized slide deck/workflow; manage waiting room, chat, and follow-up links).
· Occasionally support longevity health screening assessments (brain health scans, cardiovascular health scans, and VO2 max testing with easy-to-follow protocolized platforms)
Key Responsibilities — Pre-Visit Preparation
· Confirm labs resulted and uploaded to the medical record.
· Obtain and upload external records (consult notes, imaging, specialist reports).
· Update medications and supplements to reflect any recent changes (including dosage/brand).
· Reschedule the visit if critical labs/records will not be available; set expectations with the patient.
Key Responsibilities — Post-Visit Follow-Through
· Ensure the patient has seen and understood the clinician's note; summarize key takeaways and action items in plain language.
· Verify next steps: orders placed, referrals/radiology submitted, pharmacy/supplement orders initiated or picked up; close the loop with patients and external partners.
· Track open tasks in the EHR/task system; update statuses and due dates.
· Confirm key referrals are completed and records made available (colonoscopy, sleep study, specialist referrals)
Documentation & Quality
· Document all communications and actions within the EHR/portal following SFM SOPs.
· Contribute to continuous improvement by suggesting updates to SFM templates, FAQs, and SOPs.
Minimum Qualifications
· 2+ years in a medical assistant, care coordinator, patient navigator, or similar role (in-person, virtual, or hybrid).
· Demonstrated HIPAA/PHI compliance and professional discretion.
· Proficiency with EHR/portals, secure messaging, email, spreadsheets, and cloud docs.
· Excellent written and verbal communication in English language; confident patient education and summarization skills.
· Reliable transportation for occasional on-site tasks within Moore County.
Preferred Qualifications
· Experience in Functional/Integrative/Lifestyle Medicine or direct-care/concierge models.
· Familiarity with common functional labs (e.g., supplement brands, specialty testing labs, stool testing), supplements, and referral workflows.
· BLS certification (or willingness to obtain); Notary (nice-to-have).

KPI Examples (measured monthly/quarterly)
· ≥95% of pre-visit labs/records available ≥48 hours before visit.
· Median message response time ≤ 2 business-days.
· ≥90% post-visit action items initiated within 2 business day service level agreement (SLA).
· Patient communication quality (QA spot-checks) ≥ "meets" standard.
Compensation & Benefits
· Competitive; commensurate with experience ($18 - $24/hr range).

Compliance
· HIPAA/PHI training, confidentiality agreement, background check, and policy acknowledgments required.

· Day 30 (performance trial period): Complete HIPAA & SFM SOP training; demonstrate proficiency in pre-visit readiness and documentation; meet response-time SLA.
· Day 60: Independently run pre-/post-visit workflows; facilitate orientation group visit; ≥90% task closure within SLA.
· Day 90: Consistently hit KPIs; contribute at least one SOP or FAQ improvement; mentor incoming teammates as needed.
