Job Summary: Ensures that wellbeing of the residents is met and maintained including interaction and coordination with their families and/or significant others upon admission and throughout their time at the community. Has oversight of duties and functions of administrative assistant and/or front desk staff.

Duties and Responsibilities:

  • Plans, develops, organizes, implements, evaluates the resident and social services needs for the Manor.
  • Assists in developing, implementing and maintaining current policies and procedures not less than annually.
  • Ensures facility compliance with current federal and state regulations as they pertain to the services at the Manor by keeping them current with all regulations and rules as well as professional standards of practice.
  • Provides resource information to the resident/family to assist them in decision-making and placement.
  • Maintains confidentiality of all client/resident interactions.
  • Performs one-to-one support as needed with residents and families.
  • Conducts resident evaluation meetings regularly to meet required timelines.
  • Coordinates the initial expectations meetings for new residents in conjunction with admissions.
  • Completes billing forms to memorialize changes in level of care for residents.
  • Serves as the main point of communication, both written and verbal, between St. Paul's Manor and the resident/family regarding level of care changes.
  • Completes discharges for residents who leave the Manor and documents their end of stay in the electronic health record and via billing forms.
  • Assist with and coordinate the move-in and move-out of the apartment for any residents upon admission and discharge in conjunction with the Sales department.
  • Serves as liaison for State Licensing and completes required forms as requested.
  • Maintains a current resource file and utilizes as needed.
  • Coordinates special needs of residents with families and outside resources.
  • Visits and/or check on residents when out of the community for future discharge planning.
  • Schedule and coordinate new resident orientations, hospitality committee and Resident's' Association. Work with or assist in activities for Manor residents particularly small group interactions.
  • Has oversight of duties and functions of administrative assistant and/or front desk staff including recruiting, training and coordination of administrative activities.
  • Email newsletters and any other necessary communications to families
  • Maintain updated email distribution list.
  • Assists with special projects as requested.
  • Performs other duties as may be assigned by facility Administrator or COO/CEO. May be required to perform other resident services or administrative activity at other St. Paul's facilities when needed.
  • Must be able to communicate with, be interested in, and have compassion for the elderly. Must have good communication skills, both written and verbal, in the English language.
  • Must be able to work as a member of the interdisciplinary health care team. Must possess organizational skills to meet deadlines and goals. Able to maintain professional composure under sometimes-stressful conditions.
  • Must be computer literate sufficient to perform all tasks required of Electronic Medical Records (EMR) system. Able to work with word processing and spreadsheet application software.
  • Performs other duties as required or requested

Qualification and Requirements:
Education - A Bachelors and/or Masters degree in Social Work or a related field or equivalent field experience with seniors.

Experience – Must have a minimum of two years in a geriatric or similar setting, preferably residential care for the elderly. Must understand federal, state and local regulations pertaining to residential care and Resident Rights. Certification as a RCFE administrator preferable.
