We are seeking a Physical Therapist for a high-volume skilled nursing and rehabilitation center in San Rafael, CA. This facility offers short-term rehab, long-term care, orthopedic recovery, and therapy for medically complex residents.
You will conduct thorough evaluations, create individualized treatment plans, and deliver functional and mobility-based therapy interventions. This center offers strong interdisciplinary support, a busy caseload, and slightly higher guaranteed hours.

• Perform PT evaluations and assessments.
• Create individualized care plans tailored to patient goals.
• Deliver therapeutic exercises, gait training, and functional mobility activities.
• Modify treatment plans based on clinical progress.
• Document evaluations, treatments, and progress notes accurately.
• Collaborate closely with nursing, physicians, and therapy colleagues.
• Provide caregiver and patient education.
• Maintain compliance with regulatory requirements.

• Valid California PT license.
• SNF experience preferred but not required.
• Strong interpersonal, clinical, and documentation skills.

• Busy, supportive SNF rehab environment in San Rafael, CA.
• Higher guaranteed weekly hours (38 hours).
