Position Overview

We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our Los Angeles team. The ideal candidate is compassionate, detail-oriented, and motivated to make a meaningful difference in the lives of elderly patients. This role combines hands-on patient care with cutting-edge digital health technology, offering an opportunity to practice medicine in a collaborative and forward-thinking environment.

Responsibilities
  • Conduct clinical assessments and provide ongoing care management for patients enrolled in Happy Vitals' RPM/CCM programs
  • Review and interpret patient data from monitoring devices, ensuring timely interventions and follow-ups
  • Collaborate with facility staff, caregivers, and multidisciplinary teams to optimize patient care
  • Document visits, care plans, and patient progress in EMR systems accurately and promptly
  • Provide patient and family education on health management, prevention, and wellness strategies
  • Participate in quality improvement initiatives and clinical team meetings
Qualifications
  • Current Nurse Practitioner (NP) or Physician Assistant (PA) license in California (required)
  • Active DEA license (preferred)
  • Experience in geriatrics, internal medicine, primary care, or long-term care is strongly preferred
  • Strong communication, organizational, and clinical documentation skills
  • Comfort working with digital health tools and patient monitoring systems
  • A passion for improving healthcare delivery through innovation
