Client Care & Support

  • Serve as the main liaison for clients, families, and caregivers regarding care-related inquiries.
  • Conduct client intake, gather required documentation, and ensure all records are complete and up to date.
  • Assist in developing and updating client care plans under company guidelines.
  • Address client concerns, resolve issues promptly, and escalate matters when needed.
  • Ensure clients receive services that meet their individualized needs.

Scheduling & Staffing

  • Coordinate with employee supervisor to maintain caregiver schedules to match client needs.
  • Communicate schedule changes, client updates, and assignments with staff in a timely manner.
  • Ensure adequate staffing coverage for all shifts, including emergency schedule adjustments.

Administrative Duties

  • Maintain accurate client files, visit logs, and compliance documents.
  • Assist with keeping up with attendance, time sheets, and service authorizations.
  • Assist with onboarding caregivers and ensuring all required credentials are submitted.
  • Maintain confidentiality in accordance with HIPAA standards.

Quality Assurance

  • Assist with monitor caregiver performance and report any concerns to management.
  • Participate in audits, compliance checks, and client satisfaction follow-ups.
  • Ensure all care services follow agency policies and state regulations
  • Follow up on reports for any incidents or complaints.

Field Support (When Needed)

  • Must be willing and able to assist in the field if the agency is short-staffed or in urgent situations.
  • Provide direct client care when necessary to ensure continuity of services and client safety.

Marketing & Community Outreach

  • Represent the company professionally at community events, health fairs, and networking functions.
  • Build and maintain relationships with referral sources, partner organizations, and community groups.
  • Assist in developing and implementing marketing strategies to increase client referrals and promote agency services.
  • Support efforts to grow the company's presence in the community through outreach, distributing materials, and participating in business development activities.

Collaboration With RN Supervisor

  • Work closely with the RN Supervisor during client intake and assessment processes.
  • Assist with coordinating and preparing for the RN's initial assessment for each new client.
  • Ensure all assessment information is documented accurately and communicated to the appropriate staff.
  • Be responsible for creating and distributing a clear, accurate Plan of Care to the assigned aide based on the RN's assessment and client needs.

Qualifications:

  • High school diploma or equivalent
  • Experience in home care, healthcare administration, or a similar role is strongly preferred.
  • Strong communication, time management, and organizational skills.
  • Ability to multitask while working in a fast-paced environment.
  • Proficiency in Microsoft Office, scheduling software, and electronic health records.
  • Compassionate, professional, and customer-service oriented
