Duties
- Conduct thorough assessments of clients' needs, including mental health, addiction recovery, child welfare, and housing stability.
- Develop personalized care plans that incorporate crisis intervention strategies and motivational interviewing techniques to motivate positive change.
- Provide ongoing case management by coordinating services such as behavioral health support, social work interventions, and workforce development programs.
- Assist clients with navigating complex systems like Medicaid, Medicare, Section 8 housing programs, and child welfare services.
- Offer crisis management support during urgent situations, ensuring immediate safety and connecting clients with emergency resources when necessary.
- Mentor and supervise less experienced staff or volunteers involved in client support activities.
- Advocate for clients' rights and needs within community agencies, ensuring access to appropriate resources such as childcare and housing assistance.
- Maintain detailed case records, progress notes, and documentation in compliance with organizational policies and regulatory standards.
- Collaborate with multidisciplinary teams—including healthcare providers, social workers, and community organizations—to deliver holistic care.
- Stay current on best practices in behavioral health, social work, and workforce development to continually enhance service quality.
Qualifications
- Proven experience in crisis intervention, crisis management, or behavioral health settings.
- Strong background in addiction counseling or related fields with a focus on recovery support.
- Demonstrated knowledge of social work principles and practices related to child welfare and community services.
- Skilled in motivational interviewing techniques to engage clients effectively.
- Supervising experience is preferred to guide team members or volunteers supporting client needs.
- Familiarity with housing programs such as Section 8 and government healthcare systems like Medicare is highly desirable.
- Experience working with diverse populations including children, families, and individuals facing socioeconomic challenges.
- Excellent communication skills—empathetic listening combined with clear documentation—are essential for success.
