The Care Manager, as part of the interdisciplinary team, assess, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post hospitalization. The Care Management model provides effective transition planning and length of stay oversight to maintain patient experience, safety, and quality of care utilizing performance metrics and adoption of best practices. The Care Management model provides effective transition planning and length of stay oversight to maintain patient experience, safety, and quality of care utilizing performance metrics and adoption of best practices.
