The Perinatal Nurse Coordinator/Educator is a Registered Nurse (RN) responsible for managing, educating, and coordinating care for the panel of obstetric (OB) patients within the practice. This role bridges clinical coordination, patient education, and case management to ensure that all pregnant patients receive comprehensive, evidence-based, and patient-centered care. The Nurse Coordinator/Educator collaborates closely with each patient's managing physician and care team to promote optimal outcomes for both mother and baby.

This position combines patient advocacy, public health education, and case coordination and is integral to maintaining quality, compliance, and continuity of care across the practice.

Requirements:

Key Responsibilities

Patient Education and Compliance:

  • Meet with all pregnant patients twice during pregnancy to provide required and routine prenatal education, including Department of Health (DSHS)- mandated topics such as: Cord Blood banking information, newborn screening and information for parents of newborns, and recommended maternal vaccinations (e.g., influenza, Tdap)
  • Provide education on prenatal wellness, fetal movement monitoring, signs of preterm labor, and postpartum care.
  • Develop and maintain up-to-date educational material for patients.
  • Ensure that all required patient education is documented per DSHS and practice standards.

High-Risk OB Case Coordination:

  • Serve as the main nursing contact and care coordinator for high-risk OB patients.
  • Monitor high-risk patients' progress, ensuring follow-up visits, and specialty referrals are completed as ordered.
  • Collaborate with managing physicians and interdisciplinary teams to ensure continuity and safety of care.
  • Track outcomes and identify patients in need of additional support or intervention.

Gestational Diabetes Education and Management:

  • Provide patient education and support for gestational diabetes, including dietary counseling, glucose monitoring, and lifestyle management.
  • Coordinate with MFM to monitor glucose logs and communicate with managing physician regarding adjustments to care plans as needed.
  • Serve as a liaison between the patient and the care team to reinforce understanding and adherence.

General Coordination and Support:

  • Maintain and organized system for tracking all pregnant patients within the practice.
  • Follow up on OB patients with frequent or long missed appointments to ensure care continuity.
  • Identify patients eligible for external support programs (e.g., WIC, perinatal home visiting programs) and facilitate referrals.
  • Serve as a resource and educator for staff on obstetric care processes, policies, and patient communication.

Qualifications

Education:

  • Bachelor of Science in Nursing (BSN) required; Masters of Science in Nursing (MSN) preferred.

Licensure and Certification:

  • Active and unrestricted RN license.
  • Current Basic Life Support (BLS) certification.

Experience:

  • Minimum 2-5 years of recent clinical experience in obstetric, maternal-newborn, or women's health nursing.
  • Prior experience in patient education, case management, or coordination of care is highly desirable.

Skills and Attributes:

  • Strong knowledge of prenatal and postpartum care, high-risk pregnancy management, and maternal health education.
  • Excellent communication and interpersonal skills with patients, families, and multidisciplinary teams.
  • Strong organizational and time-management skills.
  • Compassionate, patient-focused approach with a commitment to patient education and advocacy.
  • Proficiency in EMR systems and ability to manage data for tracking and compliance.
  • Ability to work independently within a collaborative, private practice setting.
