Title: Hospice Chaplain
Schedule: 20 hours/week, within hours of Monday-Friday from 8am-5pm
Pay: $29.76 to $37.20 per hour
Location: Denver South Area. This role is primarily spent in the community, visiting patients and families in their primary residence.
JOB SUMMARY:
The Healthcare Chaplain provides spiritual guidance and support during illness, injury and/or an emergency. They will support patients and families facing difficult medical choices, and plan and coordinate interfaith worship experiences for staff, patients, and families.
EDUCATION & REQUIREMENTS:
- Master's Degree in Pastoral Care or Theology from an accredited program.
- Minimum 1+ years of chaplaincy experience in a medical setting; palliative or hospice experience preferred.
- Must successfully pass agency required background checks.
- Must possess a valid driver's license, provide proof of insurance indicating you have 100k/300k bodily injury coverage and 100k property damage coverage, and have reliable transportation.
- Must comply with agency health screening requirements.
- Must be able to demonstrate computer competency.
ESSENTIAL DUTIES:
- Identifies the patient's and family's spiritual needs and participates in individualizing the patient's plan of care based on those needs.
- Incorporates patient/family preferences and goals within the plan of care.
- Supports and respects the patient's and family's unique spiritual and cultural beliefs.
- Serves as spiritual support for staff regarding issues of grief and loss as it relates to their professional roles.
- Documents assessments, interventions, outcomes, follow-up, and communication with IDT for patient visits; completes documentation in an accurate and complete manner in compliance with regulatory standards.
- Supports facilitation of family meetings when appropriate.
- Identifies faith community resources for the patient and family; maintains a working knowledge of community resources for funerals and memorial services.
