Summary

Work under the direction of Clinical Support Supervisor. Participate in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the health center and within the parameters of the scope of practice for the Licensed Practical Nurse. Cooperate with the members of the nursing and medical staff and other health center departments.

Responsibilities

• Provide individualized nursing care to outpatients under RN supervision and collaborate with the care team to coordinate patient care, including chart prep, follow-up orders, and team huddles.
• Assist with health education for patients and families and communicate patient needs with medical providers, ensuring accurate care delivery and appropriate referrals.
• Prepare, administer, and document medications per LPN guidelines, and manage medication supplies, including drug closet organization, medication samples, and temperature monitoring.
• Perform more advanced nursing tasks as competency increases and participate in emergency care as needed.
• Maintain up-to-date, accurate patient records and update medication lists and lab results for nursing home residents.
• Monitor medical supplies, track lab results, and ensure timely orders and deliveries, including injections and patient assistance forms.
• Participate in clinic committees and complete tasks such as referrals, prior authorizations, and provider orders accurately and promptly.

Qualifications

• Completion of a formal education program in practical nursing.
• Current state licensure.
• One year of clinical practice required.
• Demonstrated clinical competence.
• Assumes responsibility for own continued personal and professional growth.
• Appropriate manner, conduct, and grooming.
• Ability to work cooperatively and communicate effectively.
• Membership and participation in Licensed Practical Nurses Association encouraged.
