*Job Title: *Licensed Practical Nurse
*Reports to: *Clinic Manager
*SUMMARY:*
Provide assistance to patients, providers and staff by providing clinical assistance.
*ESSENTIAL DUTIES AND RESPONSIBLITIES*
1. Collaborates with the provider to determine nursing needs of the patient.
a. Prepares patients for examination and treatment.
b. Provides for patient comfort.
c. Monitors physiological status.
d. Prepares exam and treatment rooms with necessary instruments.
e. Performs initial screening of patients.
f. Assists with medical treatment of patients.
g. Provides emergency first aid measures.
2. Demonstrate knowledge of clinic policy and procedures as directed by the Clinic Manager
a. Use correct procedures and techniques in providing patient care.
b. Use appropriate reference material, such as Policy Manual, Procedure Manual, Safety Data Sheets (SDS), etc.
c. Demonstrate knowledge of the use of equipment needed for rendering direct patient care.
3. Assist providers in preparing room and patients for minor surgeries and procedures including EKG’s and administering medications within scope of LPN.
4. Assists with scheduling of tests, treatments, and referrals to specialists.
5. Screens telephone calls for referral to provider
6. Recognizes and maintains the confidentiality of staff, patient and clinic information according to HIPAA regulations.
7. Provides general information on departmental policies and procedures.
8. May refill patients’ prescriptions and notify patient of test results as directed/permitted.
9. Participates in professional development activities and maintains professional affiliations.
10. Assists new employees in meeting educational needs through instruction, demonstration, and feedback.
11. Actively participates as a member of Patient Centered Medical Home (PCMH) care team(s) and supports PCMH efforts and initiatives.
12. Performs other duties as assigned, including but not limited to EKG testing, collection of blood and urine specimens, wound care, performing CLIA waived testing, dressing applications and changing, suture and staple removal, duties related to vaccine preparation, administration, and record keeping in Electronic Health Record (Centricity).
13. Actively participates in risk management activities and follows proper procedures to reduce risk and ensure patient safety.
14. Complies with WVHC policies and procedures.
15. Actively participates in relevant WVHC trainings.

*QUALIFICATIONS*
Graduate of Practical Nursing program.
Currently registered with the Indiana State Board of Nurses Registration as a Licensed Practical Nurse.
Minimum of one (1) year of experience in a similar setting preferred.
Flexible in work schedule to meet the demands of the department.
CPR certification required.
Job Type: Full-time
Pay: $22.00 - $26.00 per hour
Expected hours: 40 per week
