The Care Manager MSW provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management MSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access. This posting includes multiple openings for a Care Manager MSW Available Shifts: Full-Time, Day Candidates must reside within commutable distance of the following office location: Covenant Children’s Hospital, Texas: Lubbock Must be available to work weekends and holidays as scheduled, based on operational and departmental requirements.
