JOB SUMMARY 

Reports to the Supported Housing Supervisor with responsibility for working with members on psychiatric rehabilitation goals to foster independence in Supported Housing.

ESSENTIAL FUNCTIONS

Interact one-on-one and in group settings with members.

  1. Form relationships with members by talking with them and reading their history and assessments in the charts. Socialize with them on each shift to build rapport and a trusting, friendly bond.
  2. Know the goals of each member by reading the Recovery Plans. Become familiar with each goal and the rationale for it. Address the goals with the member during the shift. Inform the Services Coordinator of additional goals as necessary.
  3. Create learning opportunities and teachable moments with members to promote further knowledge and independence.
  4. Supervise members individually as they take medications. Discuss the medications, the proper dosage, and the time it is to be taken. Educates members on how to take their medication independently.
  5. Assist members with daily skills such as cooking, general housekeeping, hygiene, and other activities of independent living. 
  6. Encourage members to participate in community activities offered by Park Center and have the members sign up on the sign-up sheet.
  7. Ensure safety of all members by checking regularly during shifts. Know where the members are and what they are doing throughout the shift.
  8. Ensure that member rights are enforced.

Collaborate with other housing staff to provide team approach to service provision.

  1. Collaborate with Supported Housing Supervisor and Services Coordinator to provide psychiatric rehabilitation services to members. 
  2. Assist with groups and individual sessions with members as instructed by housing staff.
  3. Communicate important information to Services Coordinator via email or phone call. Progress notes are read often but not daily; therefore, some information should be communicated as soon as possible.

Maintain daily operation of living facilities.

  1. Ensure that the house is kept neat and clean by assisting members in completing assigned chores daily. 
  2. Coordinate room turnover for new members. Dispose of belongings left behind, clean room, launder linens. 
  3. Keep linens clean and neatly organized in closets or other designated areas.
  4. Maintain members' personal food in designated area, keep clean, organized, and in containers.
  5. Notify Supported Housing Supervisor of maintenance issues as they arise.
  6. Perform monthly fire drills (as assigned).

Recordkeeping Duties

  1. Document daily progress notes on each member in EHR as well as documenting house activities in the house logbook.
  2. Document when medication is given by initialing medication log.
  3. Complete night security checks of house when applicable.
  4. Complete the shift change checklist at the end of every shift.
  5. Clock in and out using the electronic system.
  6. Complete annual evaluation.

Value cultural competence and diversity 

  1. Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds. 
  2. Take deliberate steps to increase own cultural competency by attending trainings, events, discussions, workshops, etc.

Other duties as a member of Park Center Housing Staff:

  1. Participate in quarterly supervisions with Supported Housing Supervisor.
  2. Complete Essential Learning courses by deadline.
  3. Keep up with current issues and correspondence by reading emails on each shift.
  4. Assist with housing events and extra activities as applicable.
  5. Maintain good attendance and punctuality. Follow policy for vacation and sick time.
  6. Follow all Personnel Policies and Procedures.
  7. Follow HIPAA confidentiality guidelines.
  8. Follow emergency procedures (i.e.: resident crisis, injury, or weather).
  9. Follow On-Call procedure.

SKILLS AND QUALIFICATIONS

  1. A high school diploma or GED or
  2. A minimum of three years' experience in a related field
  3. Ability to work independently and under minimal supervision.
  4. Good oral and written communication skills.
  5. Strong professional and ethical behaviors.
  6. Knowledge of basic computer skills.

WORKING CONDITIONS

The position involves working with residents who have behavioral health concerns and who may at times exhibit behavioral issues. 

Potential exposure to communicable diseases including the common cold, flu, and hepatitis.

PHYSICAL DEMANDS

Must be able to sit, walk, stand, twist and bend for brief periods of time. Occasional reaching and or grasping using hand and/or arms.  May be required to move or lift up to 20 pounds. May be required to ascend/descend stairs. Regular use of hands requires dexterity in using the telephone, computer keyboard or mouse. Ability to handle disruptions due to requests from members.

SCHEDULE 

Position requires availability to work irregular hours, including evenings, overnights, and weekends.
