Licensed Practical Nurse

Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, and acute care environments. The LPN will work under the supervision of registered nurses and physicians to deliver essential nursing services, ensuring the well-being of patients while adhering to best practices and regulatory standards.

Duties

• Provide direct patient care, including administering medications and treatments as prescribed.
• Conduct patient assessments and monitor vital signs to evaluate health status.
• Assist with personal care activities such as bathing, dressing, and grooming.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Maintain accurate medical documentation in compliance with HIPAA regulations.
• Support patients with chronic conditions, including Alzheimer's care, dementia care, and pain management.
• Educate patients and families on health management and wellness strategies.
• Ensure a safe environment by adhering to infection control protocols.

Qualifications

• Valid LPN license in the state of practice.
• Experience in critical care, long-term care, assisted living, or hospital settings is preferred.
• Proficiency in basic life support (BLS) techniques and patient assessment skills.
• Strong communication skills to effectively interact with patients, families, and healthcare teams.
• Experience working with individuals with developmental disabilities or in hospice care is desirable.
• Ability to perform catheterization, tube feeding, and other specialized nursing procedures as required.
