Overview

We are seeking a compassionate, skilled, and self-motivated Home Health Physical Therapist to join our care team. In this role, you will provide high-quality rehabilitative services to patients in the comfort of their homes. You will help patients regain mobility, improve functional independence, and enhance overall quality of life through customized therapy plans.

Key Responsibilities

  • Conduct comprehensive patient assessments to evaluate physical function, mobility limitations, pain levels, and home safety.
  • Develop individualized treatment plans based on patient goals, medical conditions, and clinical best practices.
  • Provide hands-on therapeutic interventions such as gait training, strength and mobility exercises, balance training, and pain management techniques.
  • Educate patients and caregivers on home exercise programs, safety precautions, and mobility strategies.
  • Utilize clinical judgment to adjust treatment plans as patient needs change.
  • Document evaluations, progress notes, and treatment outcomes in compliance with state, federal, and agency guidelines.
  • Collaborate with physicians, nurses, occupational therapists, and other members of the interdisciplinary care team.
  • Monitor and report changes in patient status promptly.
  • Ensure all care is delivered with compassion, professionalism, and respect for patient dignity and autonomy.
  • Maintain accurate scheduling and communicate effectively with the home health agency.
  • Adhere to all HIPAA regulations and maintain patient confidentiality.

Qualifications

  • Licensed Physical Therapist (PT) in the state of practice.
  • Graduate of an accredited Physical Therapy program (DPT, MSPT, or BSPT with current licensure).
  • Previous home health experience preferred but not required.
  • Strong clinical assessment and critical-thinking skills.
  • Ability to work independently and manage a flexible caseload.
  • Excellent communication and interpersonal skills.
  • Reliable transportation with valid driver's license and proof of insurance.
